Terms of Services: Please accept this as your authorization and instructions to turn on the requested services at our Cape residence in accordance with your normal procedures as modified by any special instructions. We understand that your service is on a first come-first serve basis and our house will be opened as close to the date requested as possible. Furthermore, we understand that the requested services could be performed up to 3 weeks prior to the requested date. I understand that submitting this form electronically or physically serves as my authorization and signature.

There will be an additional $100 overtime charge [on top of your regular turn-on rate] to anyone requesting less than 3 weeks notice.

Please Note: This form HAS to be returned to our office completely filled out in order for us to properly turn your house on. A follow up phone call from you to our office will be helpful but this form is a must.
